Section 10:69-7.3 - Documentation of verification of factors of eligibility(a) It is essential that the CWA carefully document its verification of all eligibility requirements. It is extremely important that when reference is made to a document or source of verification, sufficient information be provided so that the document or source can be readily identified.(b) The preferred method of documentation is inclusion in the case record of the original document, or photocopy of such document verifying a factor of eligibility.1. If the eligibility worker has reviewed but not obtained a copy of a document, a description of the document and its location shall be included in the case record to facilitate review of the material, where necessary, by the Division of Medical Assistance and Health Services.(c) There shall be instances where the limited space provided on the application for verification shall be insufficient to record all relevant facts. It shall then be necessary to provide whatever further information is needed through narrative recording in the case record. When this occurs, the eligibility worker shall indicate on the application that additional information is available in the case record. N.J. Admin. Code § 10:69-7.3
