Section 1001.211 - Issuance and Form of License(a) The executive director shall issue a license to an applicant for a license under this subchapter if:(1) the application is submitted in accordance with this subchapter; and(2) the applicant meets the requirements of this chapter.(b) A license must be in a form determined by the department and must show in a clear and conspicuous manner:(1) the date of issuance, effective date, and term of the license;(2) the name and address of the driver training provider;(3) the authority for and conditions of approval; and(4) any other fair and reasonable representation that is consistent with this chapter and that the department considers necessary.(c) An applicant may obtain both a driver education provider license and a driving safety provider license.Tex.
