Section 3-7-31-4 - Forms used to apply for registration(a) A form used to apply for registration at a license branch must comply with 52 U.S.C. 20504(c)(2) and 52 U.S.C. 20504(d).(b) A form used to apply for registration at: (1) a public assistance agency designated under IC 3-7-15;(2) an agency serving persons with disabilities designated under IC 3-7-16;(3) an additional office designated under IC 3-7-18 or IC 3-7-19; or(4) an office of the department of employment and training services designated under IC 3-7-20.5; must comply with 52 U.S.C. 20506(a)(6).
