Section SPS 71.01 - Application for certificationAn applicant for certification as an acupuncturist who has never practiced acupuncture or who does not qualify for certification under s. SPS 71.03, shall submit to the department:
(1) An application on a form provided by the department. Note: Application forms are available on request to the department at 1400 East Washington Avenue, P.O. Box 8935, Madison, Wisconsin 53708.
(3) The fee specified in s. 440.05(1), Stats.(4) Evidence that the applicant has never been the subject of any disciplinary action by any professional or licensing authority, and subject to ss. 111.321, 111.322 and 111.335, Stats., has not been convicted of any offense substantially related to the practice of acupuncture.(5) Evidence of successful completion of the NCCAOM examination in acupuncture, with a passing score as determined by the NCCAOM.(6) Evidence of successful completion of course of study and residency, the equivalent of at least 2 consecutive years of full-time education and clinical work in Oriental diagnostic and therapeutic theories and practices at a school accredited by the national accreditation commission for schools and colleges of acupuncture and Oriental medicine or the NCCAOM.(7) Evidence of successful completion of a clean needle technique course acceptable to the department.Wis.
