Section R590-238-12 - Service Providers(1) A person may not act, in or from this state, as a captive insurance manager, broker, agent, salesperson, or reinsurance intermediary for captive business without the commissioner's authorization.(2) An application for authorization shall be on a form prescribed by the commissioner.Utah Admin.
