Section R590-146-22 - Multiple Policies Report(1) On or before May 31 of each year, an issuer shall file the Form for Reporting Medicare Supplement Policies report for each individual resident of this state for which the issuer has in force more than one Medicare supplement insurance policy or certificate.(2) The information shall be grouped by individual policyholder.Utah Admin.
