N.J. Admin. Code § 13:39-4.9

Section 13:39-4.9 - Change of name

(a) When a pharmacy permit holder intends to change the name of the pharmacy, the permit holder shall apply to the Board, at least 30 days prior to such change, for an amended permit. The permit holder shall submit a new permit application pursuant to 13:39-4.1 and the new permit application fee set forth in 13:39-1.3. The Board shall issue an amended pharmacy permit reflecting the new name of the pharmacy.

(b) The Board shall issue an amended permit bearing the new name upon return of the permit bearing the previous name to the Board for cancellation and payment of the permit fee as prescribed in 13:39-1.3.

(c) If a change in pharmacy name is associated with a change in ownership, the permit holder shall ensure that the requirements set forth in 13:39-4.5 are satisfied.
