Section 10:69-3.5 - Recording of documentation

All information, written or oral, including sources and methods of documentation, shall be recorded on Form PA-1J, Application and Affidavit for AFDC-related Medicaid and included in the case record. See 10:69-7.3 concerning documentation procedures.
