A Notice of Appeal shall contain the following:
A. The full name, address and telephone number of the person requesting the hearing;B. The full name, address and telephone number, if known, of any person whose interests could be affected by the hearing officer's decision, hereinafter referred to as an "interested party"; andC. A plain and concise statement of the reasons why the determination of the Commissioner is erroneous.223 Neb. Admin. Code, ch. 1, § 003
