Rule 30-3301-7.4 - Complaints

All complaints concerning a licensee, the licensee's business, or professional practice, shall be reviewed by the Board. Each complaint received shall be logged, recording at a minimum the following information:

A. licensee's name;

B. behavior technician and supervisor's names, if applicable;

C. name of the complaining party, if known;

D. date of complaint;

E. brief statement of complaint; and

F. disposition.
