Section 10.45.06.03 - Application

A. An application for a grant under this subtitle shall be submitted to the place and by the date and time specified in the call for proposals.

B. As specified in the call for proposals, an application for a grant under this subtitle may be required to include the following elements:

(1) A transmittal letter:

(a) Specifying:

(i) The title of the proposal;

(ii) The name of the applicant;

(iii) The name of the project director; and

(iv) A statement that the applicant agrees that submission of a proposal constitutes the applicant's acceptance of the terms of the grant program; and

(b) Signed by an individual responsible for conducting the affairs of the applicant and legally authorized to execute contracts on behalf of the applicant;

(2) A grant application cover sheet, on a form provided by the Commission and signed by:

(a) The project director; and

(b) An individual responsible for conducting the affairs of the applicant and legally authorized to execute contracts on behalf of the applicant;

(3) A statement of the applicant's contractual obligations, assurances, and certifications on a form provided by the Commission and signed by an individual legally authorized to execute contracts on behalf of the applicant;

(4) The applicant's proposal, which includes the elements specified in Regulation .04 of this chapter; and

(5) Any other information or documentation the Commission may require.
