Section 10.16.05.11 - Permit Form Requirements

A health officer shall issue a health permit that stipulates the:

A. Maximum number of spectators that may be accommodated at any one time on the premises of the outdoor musical festival;

B. Duration and times of the outdoor musical festival;

C. Location of the outdoor musical festival; and

D. Name and address of the:

(1) Applicant; and

(2) Property owner.
