Section I-4117 - Billing InstructionsA. All initial claims for the rental or purchase of DME must be filed with a statement of medical necessity. The authorized physician should supply the durable medical equipment provider with a prescription stating the medical necessity for such services with the claimant's diagnosis, prognosis, and expected time span for which the equipment or supplies will be required. The recommended form for this information is on the following page. The form should be presented to the DME Supplier by the claimant and must be attached to the initial claim form for the rental or purchase of DME.La. Admin. Code tit. 40, § I-4117
