Section 760 IAC 1-38.1-3 - "Claim" defined

Sec. 3.

As used in this rule, "claim" means a request that benefits of a plan be provided or paid. A claim may be for any of the following:

(1) Services (including supplies).

(2) Payment for all or a portion of the expenses incurred.

(3) A combination of subdivisions (1) and (2).

(4) An indemnification.
