Section 17-892.1-10 - Disclosure of information on the child(a) Information pertaining to an individual child or parents or guardians of the child shall hot be disclosed to persons other than the facility staff unless the parents or guardians of the child grant written permission for the disclosure or an emergency arises.(b) The facility shall have release of information forms available for parents or guardians to sign.(c) The parents or guardians shall be furnished with written notification of the facility's policy regarding disclosure of information.Haw.
