Rule 111-8-47-.09 - Physical Therapy Service(1) When a home has a physical therapy program, the services must be provided or directly supervised by a physical therapist.(2) A therapy record will be kept as part of the health folder on each resident receiving physical therapy. Information in the health folder shall include referral, diagnosis, precautions, initial physical therapy evaluation, treatment plans and objectives, frequency and dates of medical reevaluation.(3) The physical therapist shall keep progress notes on each resident including progress or lack of progress, symptoms noted, and changes in treatment plans.Ga.
