Section 64B9-8.012 - MediationThe Board finds that mediation is an acceptable resolution for the first instance of the following violations:
(1) Issuance of a worthless bank check to the Department or the Board for initial licensure or renewal of license, provided the licensee does not practice on a delinquent license.(2) Failure to report address changes in violation of Rule 64B9-1.013, F.A.C., provided the failure does not constitute failure to comply with an order of the Board.(3) Failure to pay fines and investigative costs by the time ordered.(4) Failure to timely submit documentation of completion of continuing education imposed by Board order.(5) Failure to update a practitioner profile within 15 days as required by Section 456.042, F.S.Fla. Admin. Code Ann. R. 64B9-8.012
