5004.1 Upon completion of the PCA Service Authorization, DHCF or its designated agent shall make a referral to the beneficiary's choice of a qualified Provider.
5004.2 A referral to a qualified Provider shall not be considered complete unless it includes all of the following:
(a) A copy of the physician or APRN's order for PCA services issued in accordance with Section 5006;(b) A copy of the completed written face-to-face assessment of the beneficiary undertaken in accordance with Subsection 5003.3; and(c) A copy of the completed PCA Service Authorization issued in accordance with Subsection 5003.4.
