Section 12.09 - Responsibilities of Applicants, Eligible Individuals, Eligible Small Employers, and Enrollees(1)Responsibility to Cooperate. The Applicant, Eligible Individual, Eligible Small Employer, or Enrollee must cooperate with the Connector or its designee in providing information necessary to establish and maintain eligibility and to bill and collect Premiums, and must comply with all the rules and regulations of the Connector or its designee. An Applicant's failure to provide information requested during the eligibility determination process may result in a delay in the eligibility determination, or a denial of eligibility.(2)Responsibility to Report Changes. The Applicant, Eligible Individual, Eligible Small Employer, or Enrollee must report to the Connector or its designee, within 30 days, changes that may affect eligibility or Premiums.
