Section 6.06 - Renewal of a Pharmacy Permit(1) Each pharmacy or pharmacy department permit issued by the Board shall expire on December 31st of each uneven numbered year following the date of its issuance.(2) Application for renewal of a pharmacy or pharmacy department permit shall be made by a duly authorized representative of the pharmacy on a renewal application form provided by the Board. Such renewal from shall be fully and properly completed and submitted to the Board in a timely manner.(3) Each renewal application form submitted to the Board shall be accompanied by a check or money order in the required amount made payable to the "Commonwealth of Massachusetts Board of Registration in Pharmacy".
