Section 2.05 - Miscellaneous Licensing Provisions(1)License Fees. Fees payable to the Board in the amount of $5.00 or more may be paid by personal check or money order drawn on a U.S. bank in U.S. funds. Fees payable in the amount of $5.00 or less may be paid by personal check. However, the Board may require any fee to be paid by certified check, money order, credit card or electronic fund transfer. The Board's fee schedule for processing various documents is set by the secretary of administration and finance pursuant to M.G.L. c. 7, § 3B. Board licensing fees are located at 801 CMR 4.02(243). The application fee is nonrefundable.
(2)Board Approval of Health Care Facility Affiliations. The Board must approve by a majority vote the affiliations between health care facilities and physician training programs, if one of the affiliations is not an ACGME or AOA accredited program. In order to approve an affiliation, the Board must determine, among other factors, that the supervision available for training purposes is adequate. Limited licensees may rotate between teaching hospitals with three or more ACGME or AOA accredited programs without prior approval of the Board.(3)Procedure for Approval of Health Care Facility Affiliations. The directors of the health care facilities and the physician training program seeking to affiliate must submit a written joint request to the Board for approval of the affiliation, at least 30 days in advance of when affiliation is sought. If the physician training program is ACGME or AOA accredited, a health care facilities affiliation agreement is not necessary.Amended by Mass Register Issue 1277, eff. 1/2/2015.Amended by Mass Register Issue 1397, eff. 8/9/2019.
