No Preferred Provider Health Plan or Workers' Compensation Preferred Provider Arrangement may be offered without meeting the filing and other requirements set forth in M.G.L. c. 152 and 1761, and until it is approved by the Commissioner in accordance with the provisions of 211 CMR 51.00.
