Section 78407 - Requirements for Participant Admission(a) Requirements for admission shall include: (1) Authorization by the Department for Medi-Cal beneficiaries which shall be initiated by the provider and shall include the results of initial assessment by the multidisciplinary team and the individual plan of care, signed by each member of the team.(2) Application by the participant.(3) Referral from community or health agencies, physician, hospital, family, friends or self-referral.(4) Residence within the service area for the adult day health center.(5) Need for the health and rehabilitative and social services provided by adult day health center in order to achieve or maintain personal independence.(b) No center shall accept participants for whom it cannot provide proper care.Cal.
