Section 1180.051 - Initiation of Audit Review Process(a) In order to initiate the Audit Review Process, the holder shall submit the Unclaimed Property Informal Audit Review Conference Request (Form AR-1 (rev. 01/2011)), herein incorporated by reference (and may be found at https://www.sco.ca.gov/upd_reporting_forms.html), to the Office of the State Controller.(b) The Unclaimed Property Informal Audit Review Conference Request (Form AR-1 (rev. 01/2011)) shall consist of at least the following information:(1) The name and address of the holder;(2) The name and title of the holder or its agent that will be participating in the Audit Review Process;(3) The audit billing notice and audit findings that were the subject of review;(4) A statement of the relief being sought;(5) A statement of the reasons and basis for review; and(6) The applicant's signature and date.(c) The holder must submit with the Unclaimed Property Informal Audit Review Conference Request (Form AR-1 (rev. 01/2011)), supporting documentation.Cal.
